Patients with dental insurance agree to assign dental insurance benefits to our office for all dental services provided for you. ANY QUOTED FEES ARE MINIMUM FEES ONLY. AS PROVIDED BY FLORIDA LAW, RESSLER DENTAL WILL CHARGE AND COLLECT A REASONABLE FEE FOR DUPLICATION OF X-RAYS, CBCT/3-D SCANS, AND/OR OTHER DENTAL RECORDS SHOULD A PATIENT REQUEST PHYSICAL COPIES BE MADE. THE PATIENT AND ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAS A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR ANY OTHER SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION, OR TREATMENT.
